Injury Report 4/11: Every Player’s DL Situation

Jeff Davis updates the massive table of all injured players in this week's edition of Injury Report.

Today’s update saw limited players entering and leaving the report, though there are a number of updates available. I’ve listed some potential scenarios for David Price and provided an update regarding Xander Bogaerts, as well as many others.

The following players have returned to regular play and have been taken off the report: Chris Davis, George Springer, Alex Wood, Gary Sanchez, Franchy Cordero, Tony Barnette, and Matt Wieters.

UPDATES

Randall Delgado (RP, Arizona Diamondbacks) – Delgado has begun a rehab assignment, and could return prior to the end of April.

Jake Lamb (3B, Arizona Diamondbacks) – Lamb has returned to baseball activities. He may not return until the start of May, but I will monitor as more updates are available.

Steven Souza Jr. (OF, Arizona Diamondbacks) – Souza Jr. experienced a setback during a rehab game. He was ahead of schedule but appears likely to be out until mid-May after all. We will get a sense of how major the setback as he moves forward.

Johan Camargo (3B, Atlanta Braves) – No setbacks – just no need to rush Camargo with Flaherty contributing like he is.

Luiz Gohara (SP, Atlanta Braves) – Gohara is slated to throw up to three innings in an outing today (4-12-18). The Braves are taking it very slowly with Gohara, but he is moving in the right direction.

Mark Trumbo (OF, Baltimore Orioles) – Trumbo started a rehab assignment but has since been shut down again. No setback has been reported, but this is still concerning and may push Trumbo back a week or two.

Xander Bogaerts (SS, Boston Red Sox) – Turns out Xander’s got an ankle fracture. A fractured talus, specifically. Typically a fractured talus means no weight bearing through that foot for 6-8 weeks – so why are the Red Sox citing a 10-14 day timetable? My guess is that when sliding down the steps of the dugout Bogaerts fractured the lateral process of his talus. This injury often mimics an inversion ankle sprain which is consistent with observations from watching the on-field exam. A 10-14 day timetable would indicate that the Red Sox are expecting the fracture to unionize quickly and without complication. Let’s hope that they’re right, as Xander is off to an incredible start to the year. However, there is a potential complication here – every process has a purpose, and in this case, the lateral process of the talus acts as an attachment site for ligaments that stabilize the ankle. If the fracture doesn’t unionize, we’ll be looking at surgery to resect the fractured piece and graft the tendon to the body of the talus – which would take about 2-3 months to recover from.

Anthony Rizzo (1B/2B, Chicago Cubs) – Rizzo hit the DL, though he expects to return when first eligible. I would not be shocked to see a similar DL stint later in the year.

Brandon Finnegan (SP, Cincinnati Reds) – Threw ~90 pitches in a rehab game but featured spotty control. I’ve moved him back approximately 5 days anticipating the Reds may want him to make one more rehab start.

Michael Lorenzen (RP, Cincinnati Reds) – Has started a throwing program. Will likely need about 4 weeks from this point. Potential to return earlier due to age and role.

Scott Schebler (OF, Cincinnati Reds) – Nerve issues are tricky, so we’re essentially playing this one by ear. The earliest he can return at this point is 4-16-18 – we’ll see how he’s doing then.

Andrew Heaney (SP, Los Angeles Angels) – Still no word if Heaney will take the ball today, which makes me think it isn’t going to happen. Perhaps over the weekend or early next week.

Ian Kinsler (2B, Los Angeles Angels) – Kinsler did not return when expected. Thursday has been tabbed as the next potential return date. No update has been provided other than “…he expects to require a couple more days”.

Matt Shoemaker (SP, Los Angeles Angels) – Inconclusive! The fun word that leaves us wondering what’s wrong with Shoe. He will reportedly undergo a follow-up nerve conduction exam in a few weeks, which means we can’t reasonably expect him back anytime soon.

J.T. Realmuto (C, Miami Marlins) – There is potential for Realmuto to return as soon as 4-16-18. He has taken at-bats in extended spring training games but will need to catch a game or two prior to returning. If he’s not back on Monday I should have a concrete timetable for you.

Christian Yelich (OF, Milwaukee Brewers) – Yelich will field-test his oblique on Friday, and may return as soon as Sunday when he is first eligible to come off the DL.

Jason Vargas (SP, New York Mets) – Vargas is reportedly not ready to return, as he has recovered slower than expected. Two weeks have been added to his estimated return date.

Aaron Hicks (RP, New York Yankees) – May return on 4-12-18. Appears very close.

ADDITIONS

David Peralta (OF Arizona Diamondbacks) – Peralta dealt with the same issue in Spring Training – the DBacks are calling it mild. The left hand indicates that it’s likely not the hook of hamate we’ve seen several guys go down with to start this year. I can’t envision this keeping him out through the weekend.

David Price (SP/RP, Boston Red Sox) – This one could have a variety of outcomes. We will know better after some testing, but essentially Price left his start on 4-11-18 with numbness in his fingers. I didn’t get to watch the game, but the wonderful folks in our community slack channel let me know that Price was seen repeatedly looking at his forearm as he left that first inning. Price is a guy who relies on his fastball and cutter to be effective. The cutter, in particular, relies on being able to feel the ball roll off your index and long fingertips. The median nerve transmits sensation for these areas. The median nerve (the same nerve that’s obstructed in carpal tunnel syndrome) runs down the inside of the forearm and can be obstructed at many points along its course. Based on the reports of numbness/tingling, and Price looking at his forearm, I’m going to take an initial guess that we have some forearm edema that has built up and caused an obstruction of this nerve. This could be as simple of a fix as regular icing and edema management techniques which could potentially have Price ready for his next start. Another possibility is that they’re not able to get the edema out – this goes on for a while and eventually, surgery is needed (see: Mark Melancon). The worst case scenario is that the median nerve is compressed as it passes near through the elbow – this can happen when the muscles that cross the medial side of the elbow tense to support the elbow when the UCL is compromised. The root is the issues is an elbow UCL sprain that requires significant management. If I had to place odds on the above outcomes it would be about 60%/35%/5%. Price will undergo testing and then we will know for sure.

Wellington Castillo (C, Chicago White Sox) – Castillo exited Tuesday’s game early and will rest on Wednesday. We have no reason to believe this is serious for the moment. Expect to see Beef in the lineup on Thursday (4-12-18).

Jordan Zimmermann (SP, Detroit Tigers) – Zimmermann was very lucky on this one. X-rays came back negative, and he passed concussion protocol. He has a chance to make his next start.

Tony Sipp (RP, Houston Astros) – Since the Astros are not referring to this as a strain and they made a retroactive DL move I could see Sipp returning once first eligible.

Alex Gordon (OF, Kansas City Royals) – The Royals think this will be a two-week absence, but they also thought Gordon was worth $72 mil/4 years. If this is any bit a significant labrum tear then I’d expect Gordon to go under the knife. I’m very curious how this will unfold.

Lorenzo Cain (OF, Milwaukee Brewers) – Cain is expected to return Friday (4-13-18). He may take things easy on the bases for a few days.

Manny Pina (C, Milwaukee Brewers) – Pina is reportedly dealing with a strained calf. Some reports have listed that he’s not likely to be ready for Friday’s (4-13-18) contest. He is to be re-evaluated and we will have a better idea of his timetable then.

Travis d’Arnaud (C, New York Mets) – This timetable will change as I anticipate the Mets and TDA opt for Tommy John. We’ll get confirmation of that within a day or two. This essentially means he’s done for 2018.

Manny Margot (OF, San Diego Padres) – Margot got hit by a pitch and suffered some bruising to his ribs. He should be fine within a couple of weeks with no residual concerns. The interesting play here is Franchy Cordero has been activated and will start in CF for the time being. Cordero was a statcast darling in his small sample last year and hit 7-for-17 in Triple-A with a HR prior to activation.

Johnny Cueto (SP, San Francisco Giants) – The Giants haven’t said a ton, but since they made a retroactive DL move I don’t think they expect Cueto to be out long. I’m leaning towards a return on 4-19-18 for now. Hopefully, this occurred on a shimmy so Cueto will cut that non-sense and stop ruining pitching.

Doug Fister (SP, Texas Rangers) – Minimal information provided, but I’m assuming it’s a grade 1 strain. The Rangers expect Fister to return in 3-4 weeks.

Elvis Andrus(SS, Texas Rangers) – Sorry he didn’t make the table – Andrus decided to get injured after the completion of this article. After X-rays Andrus was diagnosed with a fracture – I’m not sure which bone yet, but that information will guide our timetable. Broken bones take approximately 4-6 weeks to heal without complications – Andrus will then need to regain strength and any range of motion. I wouldn’t expect him to grace your lineup for at least 2 months.

Rougned Odor (2B, Texas Rangers) – Odor injured his hamstring last Thursday (4-5-18) and then re-aggravated it this week. I’d expect the Rangers to be patient since they’ve already tried to rush this once with poor results. Let’s face it, Odor’s .565 OPS to start the year isn’t something you NEED to have in the lineup. No need to rush.

Kendrys Morales (1B, Toronto Blue Jays) – The Jays don’t anticipate a minimum DL stay. The provided timetable is two weeks, but due to Morales’ size, age, and injury history, I’m going to guess he’ll miss 3-4 weeks.

Adam Eaton (OF, Washington Nationals) – It seems as though Eaton came back too soon. While the X-ray came back negative I’m curious what the MRI may indicate – he reportedly got an MRI on Wednesday (4-11-18) and the results should be reported on within 24-48 hours. For now, the Nats are calling it a bone bruise. The current plan is for Eaton to rest for an entire week prior to returning to baseball activities.

Jhonatan Solano (C, Washington Nationals) – Solano was called up to cover a paternity leave and sent to the DL a few days later. The timetable begins whenever he undergoes surgery to remove the bone spurs.
